Dr. Farhan sees patients through CLS Health, which accepts a wide range of insurance plans. Insurance coverage varies by plan. Our team can assist with insurance verification and authorization requirements when necessary.
Many patients can schedule directly with Dr. Farhan. Certain plans may require a referral or prior authorization — our scheduling team can help determine the requirements for your individual plan before your visit.

Self-Pay, Billing & Financial Assistance
For patients without insurance or those seeking services not covered by insurance, our office can discuss self-pay options and payment expectations. Financial concerns should never stand in the way of necessary urologic care.
Billing is administered through CLS Health, which works with several medical billing companies. Questions about a statement are best directed to the billing company listed on that statement. For general account questions, the CLS Health customer service center can be reached at 281-724-1860, Monday through Friday, 8 AM to 5 PM.
CLS Health does not publish a single third-party financing partner. Self-pay pricing, payment arrangements, and any available financing are confirmed case by case. You may also request a written cost estimate before tests or procedures are performed. Please call 346-414-3426 for current details.
You are also protected from surprise and balance billing for emergency care and for many services delivered at an in-network facility. Details are available in the CLS Health patient resources library.
